What is Hospital @ Home / Urgent Community Response?
Hospital @ Home and Urgent Community Response (UCR) is a service provided by Frimley Health NHS Foundation Trust.
It allows eligible patients to receive hospital-level medical care in the comfort of their own home, avoiding the need for hospital admission wherever it is safe to do so.
Our team can provide the same day medical assessment, investigations and treatments that you would receive in hospital, whilst supporting you to remain at home.
How long will I receive care?
Most patients are cared for by the Hospital @ Home / UCR team for up to 7 days.
However, some people may only need our support for one or two days, depending on their condition.
What happens after I am referred?
Once you have been accepted onto the service, depending on the urgency, we will see you within 2 hours or during our working hours same day next day.
Our service operates 7 days a week between 08:00am and 20:00pm. Unfortunately, we cannot offer specific appointment times.
During your first visit, a member of our clinical team will:
- Carry out a full medical assessment
- Check your observations
- Take blood tests if needed
- Perform an ECG if required
- Discuss your treatment plan with you
If you need further investigations, such as a x-ray or scan, we will arrange these at your local hospital.
How will my care be managed?
- Your care will be reviewed daily.
- You will have contact daily with us either via a visit or telephone call, depending on diagnosis.
- If your treatment or care changes, we will discuss this with you.
When will I be discharged?
Once you are medically well enough, you will be discharged from the Hospital @ Home / UCR service and your ongoing care will return to your GP or usual healthcare team, just as it would following hospital stay
What should I do if I become more unwell?
The Urgent Community Response (UCR) team is available 7 days a week from 08:00am to 20:00pm
