We are so sorry for your loss.
Please accept our sincere condolences on the loss of your baby. We understand this is a very difficult time for you and your family.
You are getting this leaflet because we want to explain how we review the care you received. When a baby dies unexpectedly, it is always reviewed and investigated. This is a national rule and helps us learn and improve care.
- If your baby was born at or after 37 weeks, died soon after birth, and meets certain criteria, your care will be reviewed by an independent organisation called Maternity and Newborn Safety Investigations (MNSI).
- All other cases where a baby was over 22 weeks will be reviewed by our maternity team.
- Your medical notes will be checked carefully.
- Staff may be interviewed.
- A panel of senior doctors and midwives will meet to discuss your care.
- If care could have been better, we will make a plan to improve.
We use a tool called Perinatal Mortality Review Tool (PMRT). This is used across the UK to help reduce baby loss and improve care for families.
Yes. Your feedback is very important and helps us improve care. You will get a letter inviting you to share any questions for the PMRT review.
You can:
- give questions to your midwife before you go home
- or contact the Lead midwife for patient safety 0300 613 6272 or Bereavement midwives 07824 321964 later.
- Most reviews take about 12 weeks.
- Some may take longer if they are complex.
- If your case goes to the Healthcare Services Investigation Branch, you will get your report after their review is finished.
Yes. We can give you a copy and go through it with you. If the report is ready when you see your consultant, that is a good time to review it together.
