Paediatric ECG requests: guidance
From 5 October 2026, Frimley Health NHS Foundation Trust (FHFT) will begin sending completed ReSPECT (Recommended Summary Plan for Emergency Care and Treatment) forms electronically to GP practices via Docman. This is an interim step as we work towards a longer-term solution of shared digital forms.
What is changing?
ReSPECT forms completed within FHFT will now be created within the Trust's Epic electronic patient record and, from 5 October 2026, a copy of the completed form will be sent to the patient's registered GP practice via Docman.
What does this mean for GP practices?
- A completed ReSPECT form will be sent as a separate document via Docman when the patient is discharged.
- The document will be titled ‘HIM Release of Information Output’ (see screenshot below):

- A statement confirming that a ReSPECT form has been completed will also be included within the discharge summary (see screenshot below):

- Practices may wish to code the presence of a ReSPECT form as an active significant problem using the appropriate SNOMED code (see screenshot below). The document should otherwise be managed in line with normal procedures for FHFT correspondence.
- SNOMED Codes:

- Patients will continue to receive a paper copy of their ReSPECT form on discharge. An electronic version will also be available to the patient through the MyFrimleyHealth app and to authorised clinicians through Epic Care Link (Chart Review → Media, see screenshot below)

Important points:
- This electronic document is intended to ensure GP practices receive a copy of the patient's FHFT ReSPECT form.
- There is no expectation that practices manually transcribe information from the document into Connected Care.
- Receipt of an FHFT ReSPECT form will not automatically overwrite or update any existing ReSPECT form held within the GP record or Connected Care. Clinical judgement should be used where multiple versions exist.
