All IVAS referrals for Frimley Park and Wexham Park Hospital should be submitted via the designated eRS service (details below) and not via email or via the ‘Haematology - General (Not IVAS) - Frimley Health’ eRS service.

For GPs facing Wexham Park Hospital an additional Wexham location has been set up for this service which will be live on eRS from Monday 16 November. Until then, please select Frimley as the location in your search to find the service and the patient will be triaged and booked to the correct site.

To support triaging assessment and prescribing, it is essential that all referrals include blood test results (haemoglobin, ferritin, serum iron, transferrin and transferrin saturation) and a recorded patient weight, both obtained within the last four weeks.

The DXS IVAS (Intravenous and Vascular Access Services) referral form called ‘FHFT Intravenous Iron Infusion Service Referral Form’ has been updated to include these eRS details, as well as inclusion and exclusion criteria. The form can be found in the following location on DXS/Frimley Health and Care / Iron Transfusion Landing page or via the following search terms: FHC1061 and Iron. This information has also been transferred to Ardens for Bucks practices.
