The Medical Examiners team may need to urgently contact the clinician responsible for completing the Medical Certificate of Cause of Death (MCCD). Providing a direct contact number will help the team reach the appropriate clinician promptly, without needing to call the practice’s main telephone line. 

To support this, the referral form has been updated to allow the referrer to manually enter a direct contact number in the ‘Doctor(s) able to write the MCCD’ field. This field is mandatory and cannot be left blank.
