We continue to receive a high number of Rapid Access TIA referrals by email. To ensure referrals are received, tracked and reviewed promptly, please submit all FPH-service referrals through eRS, using the eRS service details below.

The referral form is available on DXS within Frimley Health and Care / Stroke / Frimley ICS Referral Forms. You can also find it by searching for “TIA” or “Stroke”.

Before referring, please note the following:

  • Ensure this is a referral to Frimley Park Hospital (FPH). There is no TIA service at Wexham Park Hospital (WPH). WPH-facing practices should continue to use the High Wycombe TIA service form. This is located on DXS in the Frimley Health and Care /Stroke / East Berkshire / Referral Forms folder, please refer to: buc-tr.tiareferralwycombe@nhs.net.
  • Check the inclusion and exclusion criteria on the form. These are also available on the eRS Directory of Service (DoS).
  • Telephone the Stroke Consultant or Stroke Coordinator on 07887 293 695 to discuss the referral in the first instance or if you are seeking urgent clinical advice. The line is managed by the front door consultant / SPR / ANP and is staffed between 09.00 – 17.00 Monday to Friday. Out of hours, please call the Stroke Clinical Nurse Specialist for support on 0300 613 9331 / Bleep 5635. Please do not share these numbers with patients (Urgent-Advice-Stroke).
  • Please note, in some cases, we may ask you to redirect the referral or ask to see the patient urgently. Patients who have had a TIA need to be seen within 24 hours of first contact with a healthcare professional, UNLESS the episode is already more than 7 days ago. Please send your referral via eRS as soon as possible after your assessment to avoid any delay.
  • If you are considering sending a patient to ED with a possible TIA, please first call 07887 293 695 to discuss. For emergency stroke admissions please continue to call the 0300 613 9331 number prior to referring (Urgent-Advice-Stroke).
  • Full details of the Urgent Clinical advice lines, including the contact details and opening times referenced above, can be found on the FHFT GP centre website: https://www.fhft.nhs.uk/gps/urgent-clinical-advice#Urgent-Advice-Stroke

 Appropriate (inclusion criteria):

  • Do NOT use ABCD2 score as it is poor at discriminating risk of stroke after TIA.

 Inappropriate (exclusion criteria):

  • Gradual onset or spread of symptom
  • Seizure or loss of consciousness
  • Transient Amnesia
  • Isolated Vertigo and no other Cranial nerve features are unlikely to be a TIA. Consider alternatives referral route e.g. to the most relevant medical specialty.
