This is a reminder that all birth review referrals must be completed by a health care professional using the MS Office Form: Birth review referral form and cannot be self-referred by the women or their families. 

If the woman meets the agreed referral criteria, a Birth Review appointment will be arranged. Please note that, following clinical review, not all referrals will result in a Birth Review appointment. 

Eligibility and referral criteria 

Women who have given birth at Frimley Health within the last 12 months may be eligible if one or more of the following apply (this list is not exhaustive): 

  • Emergency caesarean section 
  • Difficult instrumental delivery 
  • Obstetric anal sphincter injuries (third or fourth degree tears) 
  • Obstetric emergencies (including postpartum haemorrhage, shoulder dystocia, eclampsia, cord prolapse, maternal collapse, or ITU admission) 
  • Intrauterine death, stillbirth, or neonatal death 
  • Neonatal unit admission due to intrapartum asphyxia 
  • Unexpected return to theatre 
If referral criteria are not met 

All referrals are individually reviewed by the Maternity Clinical Governance team. Where a referral is not accepted including where they do not meet the agreed criteria, the woman will be informed in writing and provided with information about alternative national support services.
