You can make a nomination using the online nomination form. Please include details about what this individual or team has done that goes beyond what is expected in their role, including evidence and specific examples. Your nomination should not exceed 300 words.

Nominations close at 9am on Monday 1 September

The awards will be presented during a ceremony in November 2025.

How to make a nomination

This is your opportunity to recognise the exceptional contributions of our staff members and teams who go above and beyond in delivering outstanding care and support within the trust.

Below you’ll find all the information you need to nominate someone for an award, including step-by-step guidance on the nomination process and descriptions of each award category.

Nominations should focus on the achievements of a person or team in the period from September 2024 to July 2025. There is no limit on the number of nominations you may make across the categories.

Individuals and teams cannot nominate themselves.

Nominating someone for an award is simple:

  1. Review the award categories: Take a moment to familiarise yourself with the different award categories listed below. This will help you identify the most appropriate category for your nominee.
  2. Prepare your nomination: Think about why your nominee deserves recognition. What makes them stand out? What impact have they made? You will need to summarise this in up to 300 words.
  3. Complete the online nomination form.
  4. Select the appropriate award category.
  5. Provide your details and your nominee’s name, role, and department.
  6. Write a detailed explanation (up to 300 words) of why you believe your nominee deserves to win, addressing the specific criteria for the selected category.
  7. Submit your nomination: Once you’ve completed the form, simply hit ‘submit’. You will receive a confirmation email once your nomination has been successfully submitted.

Award categories

Below are the award categories, along with a description of what the judges will be looking for. Choose the category that best fits your nominee’s achievements.

We are asking you to identify and recognise individuals or teams who make a genuine difference to the quality of care and wellbeing of our patients, their families, carers, or our staff, and who consistently contribute to delivering service improvement and efficiency in line with the trust’s values: committed to excellence, working together, facing the future.

Patient/public nomination only: 

This award gives patients, carers, relatives and visitors the chance to recognise a member of staff or team who has made a real difference. It celebrates those who go above and beyond to provide outstanding care, compassion and support.

For a team or individual who has made things work better, saving time, money or effort. Through smarter ways of working, digital tools or role redesign, they’ve boosted productivity and delivered real value to the Trust.

Savings and/or return on investment must be demonstrated.

Awarded to an individual, team or staff network who actively promote equality and inclusion. They challenge discrimination, value everyone’s contribution, and create a workplace where everyone feels respected and supported. This could include working with patients and service users or supporting colleagues through staff networks. 

A clinical or non-clinical individual with outstanding compassionate and inclusive leadership skills, inspiring people to go the extra mile. Good leaders are not necessarily managers but they will always demonstrate our trust values. They are innovative and value their peers while actively mentoring and encouraging people they work with. They are tireless in their efforts to make improvements for the benefit of all.

For someone new to the organisation or their role (less than a year) who’s already making waves. Whether through brilliant patient care, supporting colleagues, or fresh ideas, they’ve made a strong and lasting impression.

A clinical or non-clinical team that works brilliantly together to deliver high-quality, efficient services. They embrace change, share learning, and find new ways of working that make a real difference to patients and colleagues.

For someone who leads the way with bold ideas and fresh thinking. Through education, research or innovation, they’ve introduced something new that improves care or transforms how we work, and they inspire others to do the same.

This award honours a clinical or non-clinical colleague whose work often goes unnoticed but makes a big impact. They’re always ready to help, go the extra mile without being asked, and live the trust’s values every day, without expecting recognition.

Celebrating an individual or team of volunteers who give their time, energy and heart to support patients, families and staff. Their dedication helps make our services more welcoming, compassionate and effective.

This award category is for nominations made by staff members only.

Celebrating a team or individual who has used the Frimley Excellence (FX) approach to drive meaningful change. Whether improving patient safety, care quality or staff experience, their work shows clear, positive impact.

Nominations must show clear evidence of the difference made.

Tips for writing a strong nomination
  • Each award has specific criteria. Make sure your nomination highlights these criteria.
  • Explain how your nominee meets these criteria. Describe their actions and how they made a difference.
  • Share specific examples of their efforts.
  • Be detailed: Instead of saying "They are great" say "They helped reduce patient wait times by 20% by organising the schedule better".
