Pregnancy notification forms 

Frimley Health offer full maternity care at Frimley Park Hospital and Wexham Park Hospital.

If you would like to have your baby with us please complete the online pregnancy notification form for the hospital you would like to attend. We encourage you to do this as early as possible. You do not need to see your GP first.

Please select the correct link for the hospital you wish to give birth in. 

If you are considering a home birth please select the link to your nearest hospital and you can discuss this further at your booking appointment with the midwife.

If for any reason you are unable to complete the form please either print a copy or contact your GP surgery who will pass the printed form or your details to the midwife.

Antenatal Screening Tests

We encourage you to notify us as early as possible to ensure we are able to offer you appropriate scans and screening tests at the correct time

Folic acid

It is recommended that you start taking Folic acid (0.4mg) supplements as early in pregnancy as possible. These tablets are widely available in pharmacies and supermarkets. Folic acid is important for the development of your baby’s nervous system.

Vitamin D

The Department of Health also recommends that all adults, including pregnant and breastfeeding women, need 10 micrograms (mcg) of vitamin D a day, and should take a supplement containing this amount.
